Help keep Dressage Shows on Long Island! Entry Form only (Fill in online and print) This entry form can be filled in online and then printed OR you can print it out and fill it in by hand. To fill it out online, move your mouse to desired area and left click, then type. This will work with the drop down menus as well. Either way, a COMPLETED FORM must be mailed in with a check and ALL membership cards and a negative Coggins test. All incomplete entries will be charged an additional $35, in accordance the USEF rules. A handy checklist is HERE (PDF).
